Care Coordinator – Healthcare

Salary: £28,000 per annum

Location: Wimbledon, London (SW19)

Hours: Full-time | Monday to Friday | Office-based

No Weekends. No On-Call. No Covering Care Shifts.

You'll play a key role in ensuring people with complex healthcare needs receive consistent, high-quality care in their own homes.

What will you be doing?

• Coordinating care rotas and ensuring consistent staffing for clients.

• Managing a portfolio of clients and maintaining regular communication with families and carers.

• Building positive relationships through regular client contact and visits.

• Supporting carers through ongoing supervision and annual appraisals.

• Proactively resolving staffing challenges to maintain continuity of care.

• Working closely with clinical and operational teams to deliver excellent service.

• Keeping clients, families and carers informed of any changes.

This is an office-based role with client visits. There are no weekend shifts, on-call responsibilities or requirements to cover care shifts in the community.

What are we looking for?

• Previous experience in care coordination, domiciliary care or a similar healthcare or social care role.

• Experience managing care rotas, staff scheduling and coordinating care packages.

• Excellent communication and relationship-building skills.

• Strong organisational skills and attention to detail.

• Confidence communicating with clients, families and care teams.

• A proactive, flexible approach to problem-solving.

• A genuine commitment to delivering outstanding person-centred care.

Experience within complex care, domiciliary care or community healthcare would be particularly valuable.
